The Case Manager/RN Navigator is responsible for the comprehensive assessment, planning, implementation, and evaluation of individual patient needs for discharge planning, admission to swing bed and case management including utilization review in accordance with Providence philosophy, objectives, policies, procedures and standards. The Case Manager/RN Navigator partners with the Utilization Review nurse, patients, physicians, and interdisciplinary team to ensure authorization from third party payors secured; assure resource access and function as team member providing coordinated admission and discharge planning. Fosters change and teamwork to ensure constructive relationships and growth, while maintaining a positive and professional image when interacting with patients, their families, physicians, employees and general public.

Schedule is part time, 0.5 FTE, Day shift, Monday-Friday 8am-12pm in AKT zone, 4 hours per day; total 20 hours per week.
