Job Description Summary

Reporting to the System Director of Case Management, the SCDC RN Coordinator is responsible for overseeing the coordination of all healthcare services for patients in the South Carolina Department of Corrections (SCDC) system at Chester and within the Regional Health Network (RHN). The SCDC RN Coordinator will work to build trusting relationships and ensure that patient movement is a seamless process performed in a safe, timely, and appropriate manner. This includes managing transportation logistics, patient admissions and discharges, clinical updates, and facilitating appointments. The RN SCDC Coordinator is the primary contact for DOC prisoners at both MUSC Charleston and Prisma Health, working within the operational requirements of SCDC to ensure patients' safe and timely movement. The role requires effective communication, prioritization of services, and collaboration with various healthcare providers, including the MUSC network.

The RN SCDC Coordinator is the primary contact for DOC prisoners at both MUSC Charleston and Prisma Health, working within the operational requirements of SCDC to ensure patients' safe and timely movement. The role requires effective communication, prioritization of services, and collaboration with various healthcare providers, including the MUSC network. Other duties as assigned.

This is a remote work position with monthly onsite/in-person work from MUSC Chester. SCDC patient needs may include weekend service needs and coordination.

Minimum Training and Education: Bachelor's degree in Nursing from an accredited school of nursing required. 2 years of bedside nursing or case management work experience preferred.  Proficient in Microsoft Suite Solutions. Must have effective oral, written, and interpersonal communication skills, and ability to multitask.

Required Licensure, Certifications, Registrations: Licensure as a registered nurse by the state of South Carolina or compact state required. Certification in Case Management preferred. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required.
